Welcome to Your 12-Week Program

Your program includes three 28-day treatment cycles of physician-led weight-management care. Your treatment plan, medication choice, prescriptions, and dose changes are based on your medical needs, eligibility, response, and tolerance.

Your Program Option

Your signed agreement identifies the pathway you selected. Program fees are charged every 28 days for three consecutive cycles.

Program Pathway Medication / Notes
Compounded Semaglutide Program Any medically prescribed dose; standard shipping to the medical office included.
Compounded Tirzepatide Program Any medically prescribed dose; standard shipping to the medical office included.
Brand-Name or Other Medication Management Medication is prescribed but purchased separately through your pharmacy.

What Your Program Includes

Initial medical evaluation and treatment planning.

Office or telehealth follow-up visits and required monthly clinical check-ins.

Standard baseline and monitoring laboratory tests ordered through the program.

Medication selection, prescribing, dose review, and side-effect assessment.

Weight, progress, nutrition, activity, and maintenance guidance.

Coordination with your primary care provider when appropriate.

Maintain healthy, radiaStandard shipping of eligible compounded medication to the medical office for the compounded semaglutide and tirzepatide pathways.

About Included Lab Work

The program includes standard baseline and medically necessary monitoring studies. Additional testing related to an abnormal result, new symptoms, pregnancy concern, an unrelated condition, emergency evaluation, or specialist recommendation may result in a separate charge.

Billing

Your 12-Week Commitment & Billing

  • Your commitment is 12 weeks, paid in three consecutive 28-day treatment cycles using your authorized payment method.
  • This is a direct-pay program. Program fees and compounded medication are not submitted to insurance by the practice.
  • Keep a valid payment method and current billing information on file. Services and medication orders may be disrupted or delayed if payment cannot be processed.

Your Participation & Clinical Eligibility

  • Complete required visits, monthly check-ins, and ordered laboratory tests.
  • Report material health changes and side effects to the clinical team.
  • Follow dosing, injection, and storage instructions exactly.
  • Prescriptions, refills, dose increases, and continued participation are not guaranteed. Treatment may be delayed, withheld, changed, or discontinued when monitoring is incomplete or treatment is not medically appropriate.
  • No specific amount or rate of weight loss is guaranteed. Weight change may not be steady or linear.

Medication Pathways & Changes

Compounded semaglutide or tirzepatide

  • Your stated program price applies at any medically prescribed dose during the applicable 28-day cycle.
  • Dose increases are not automatic and occur only when clinically appropriate.

Brand-name GLP-1 or other prescription medication

  • The program fee covers medical management only.
  • Medication cost, pharmacy copay, deductible, coinsurance, and other pharmacy charges are your responsibility.
  • The practice does not guarantee insurance approval, coverage, pharmacy inventory, manufacturer savings eligibility, or a particular medication price.

If your pathway changes

  • The new program price takes effect on the next 28-day cycle after written confirmation of the change.
  • Medication already prepared, dispensed, or shipped is nonrefundable and does not receive a credit.

Compounded Medication & Delivery

  • Compounded medications are prepared by a compounding pharmacy pursuant to a prescription and are not FDA-approved products. They have not undergone FDA premarket review in the same manner as commercially manufactured FDA-approved drugs.
  • Use only the concentration, syringe, and injection amount shown in your written instructions. Do not convert milligrams, milliliters, or syringe units without guidance from the clinical team.
  • Eligible compounded medication is shipped by the designated pharmacy to the medical office, and standard shipping is included.
  • If a different delivery method is specially requested and approved by the medical team, you are responsible for providing an accurate address, monitoring the shipment, promptly retrieving it, following storage instructions, and reporting visible damage, delays, or temperature concerns.
  • Medication already compounded, dispensed, or shipped is nonrefundable. Replacement or expedited shipping costs may apply when loss, damage, or delay results from an incorrect address, delayed retrieval, or circumstances outside the practice’s or pharmacy’s control.

Cancellation, Medical Discontinuation & Nonrefundable Charges

  • The physician may modify or end treatment for safety, medical ineligibility, pregnancy, adverse effects, nonparticipation, unsafe medication use, repeated missed visits, incomplete monitoring, or nonpayment.
  • If the physician determines that continued medication is medically inappropriate, future medication charges stop for medication not yet ordered. You and the physician may discuss another program pathway or early conclusion of the program.
  • If you request a non-medical cancellation before completing the 12-week commitment, you may be assessed a $195 early-termination charge for one 28-day cycle.
  • You will not be charged for future medication that has not been ordered, compounded, dispensed, or shipped.
  • Fees for completed services and medication already ordered, compounded, dispensed, or shipped are nonrefundable.

Coordination of Care & Your Responsibilities

  • The practice may communicate with your primary care provider and other treating clinicians when reasonably necessary for treatment coordination, subject to applicable privacy laws and required authorization.
  • This program focuses on weight management and does not replace comprehensive primary care, urgent care, or emergency care.
  • Continue routine care with your primary care provider and seek immediate medical attention for urgent symptoms.
  • Do not share, sell, or transfer medication. Keep medication and supplies secure and away from children and other persons.

Your Monthly Check-In Matters

Your required visits, check-ins, laboratory monitoring, and side-effect reporting help the medical team determine whether your current medication and dose remain appropriate. A refill or dose increase may be delayed if required monitoring is incomplete.

At the End of 12 Weeks

At the completion of your third 28-day cycle, your progress and treatment plan can be reviewed with the medical team. Continued treatment, a pathway change, or conclusion of the program will depend on your clinical needs and treatment goals.
